The only outright "mistake" we noticed regarding the DS9/NEM "captain vest" over the years was during Deep Space Nine's mid/late sixth season episode, "In the Pale Moonlight."
During that episode, the Sisko's vest trim clearly yet inexplicably changed construction while he was monologuing!
As you can see, at first, the top of the zipper trim met the neckline trim properly:

However, the trim switched to an "improper" construction in which the neckline trim wrapped over the zipper trim!

As you can see, the zipper trim did not extend all the way to the top of the vest's center front, as it should have, but the neckline trim wrapped over it so the seam between the zipper trim and neckline trim was horizontal, rather than vertical.
